Prepares MFB cap on DI cases in a string table.

    double capt, 
    double amepia, 
    double mfbt
);

Parameters	Description
double capt	Percentage cap for DI MFB.
double amepia	Ame or pia.
double mfbt	DI MFB.

This is called from piamfb1, piamfb2, and mfbOut.
