OAO branch support staff will prepare an appeals file on all request for review cases. The appeals file will be annotated with the claimant's Social Security number.
OAO branch support staff will also prepare an appeals file for all protest and unappealed review cases and in those OQA referral cases in which the Appeals Council takes own motion review.
The appeals file will contain:
A copy of the request for review (Form HA-520), OQA referral or protest memorandum, as appropriate.
A copy of the hearing decision or dismissal order.
OAO working papers (including case analyses, audit notes, etc.), copies of correspondence, Appeals Council orders, decisions, etc.
When forwarding cases to the Appeals Council, the analyst will ensure that the route slip is stapled to the front of the appeals file, which will then be banded on top of the claim file. Place action documents for signature by the AAJ or Appeals Officer in the appeals file.
