The Electronic Records Express website provides a variety of functions to facilitate the delivery of medical evidence (from providers to DDS/OHA offices) and the exchange of general evidence and information between individual users and organizations. Not all users have access to all the website functions. In general, if the link for the function appears on the home page, it implies that you have been authorized to use that function. Please contact your Professional Relations Officer (PRO) to request access to functions for which you are currently not authorized.

Access Electronic Request
This function is a part of a feature labeled "Electronic Outbound Request" in which (CE and MER) request letters are sent in an electronic means (via the website) from the requesting office (DDS/ODAR) to the provider (hospital, doctor, copy service, etc). Through this link, the provider has the ability to view outstanding electronic requests, download request letters and other supporting documentation, and conveniently submit responses without having to enter any barcode index information.

Access Doctor's Electronic Request
This function is also a part of the "Electronic Outbound Request" feature and supports the "CE Admin Upload" feature. This link allows a Doctor's administrative staff to access their Doctors' CE electronic requests and prepare submissions.

Send Transcription Report to Doctor
Allows medical transcription services to submit transcribed medical reports to Doctors and DDS offices. Submitted files will be stored online (within respective website Inboxes) and can be accessed only by the recipients through the Pickup Files function.

Pickup Files
Allows users to access their Electronic Records Express Inbox to view and download received files.

Teacher Questionnaire
Form SSA-5665 (SSA Teacher Questionnaire) is available for download in Microsoft Word document format from the website.

Track Status of Submissions
Allows users to view the status of their submissions. Users should select this link if they want to know whether or not their submission has reached the end site or if the submission is still processing or has an error.

Customer Status Inquiry
Allows Administrators, Regional Administrators, and Sponsors to search for and view the status of all ERE submissions.
