The Nebraska State Department of Public Welfare was among the initial
carrier selections. However, the agreement was terminated by mutual
consent effective May 5, 1967, following a request by the Department
of Public Welfare.
{2} As of July 1, 1967 the Oklahoma Department of
Public Welfare assumed the carrier role for those individuals included
in the "buy-in" agreement.
