Subject: Information Disclosure

Background

This transmittal introduces new sections I-1-4-5 and I-1-4-6 in the Hearings, Appeals and Litigation Law (HALLEX) manual chapter I-1-4. Section I-1-4-5 defines personally identifiable information (PII) and section I-1-4-6 provides instructions regarding notification and offer of credit monitoring after a PII loss incident.

Explanation of Content and Changes

I-1-4-5 – We added a new section titled “Personally Identifiable Information (PII).” In this section, we define PII and provide links to the instructions for reporting a loss of PII.

I-1-4-6 – We added a new section titled “Notification and Offer of Credit Monitoring Service After Personally Identifiable Information (PII) Loss Incident.” In this section, we provide general information about submitting an incident report. We identify the individual(s) with the authority to approve remediation for PII loss incidents in the Office of Disability Adjudication and Review (ODAR). We also explain how an ODAR manager can obtain approval to notify affected individuals of credit monitoring remediation and initiate the credit monitoring blanket purchase agreement when approval is obtained.
