This website provides different types of functions to our registered users. The Evidence Functions provide online ability to respond to requests for evidence, receive requests for evidence, and check the status of submissions. Some functions are provided to every user while other functions apply only to specific users.

NOTE: You may not see all the links when you access your ERE website homepage. The links are dynamic based on the access you have been granted.

Access Electronic Requests
This function is also referred to as "electronic Outbound Request" or "eOR." Through the Access Electronic Requests option, request letters are sent through the ERE website from the requesting DDS to an ERE user. Users with this function can view electronic requests, download request letters and other supporting documentation, and conveniently submit responses without having to enter any information from the barcode. We may also provide the ability to view and submit payment requests for electronic requests after submitting a response.

Access Provider's Electronic Requests
This function is part of the "electronic Outbound Request" or "eOR" feature. This link allows a provider's administrative staff to access their provider's CE electronic requests and prepare submissions. Additionally, some administrative staff will be able to submit payment requests on behalf of the provider.
NOTE: You may not see the links allowing you to submit payment requests. The links are dynamic based on the access you have been granted and the DDS that originated the request.

Send Individual Response
This link allows you to submit evidence for a single individual. You can also submit a response to a request for records that indicates you have no records to submit.

Send Grouped Response
This link allows you to submit evidence for multiple cases and multiple evidence requests in a single bulk upload.

Send CE with Scanned Signature
This link allows providers or administrative staff to submit CE reports. The provider physically signs a hardcopy of the CE report, scans the signed copy of the CE report, and submits the CE report through this function.

Send CE Report
This link allows providers to upload reports in response to CE requests. The "Send CE Report" function allows the provider to certify the submission online via an electronic signature on an auto-generated signature attestation document.

Send CE No Show Response
This link allows providers and administrative staff to submit a response to a CE request indicating that the patient did not show for the CE.

Prepare Report for Provider
This link allows a provider's administrative staff to prepare a CE report and send it to the appropriate provider for review and submission to the requesting office.

Review/Submit Prepared Requests
This link allows providers to review and submit CE submissions that have been prepared by their administrative staff.

Track Status of Submissions
This function allows users to view the status of their own submissions.

Submission Inquiry
This function allows ERE Administrators, Regional Administrators, and Sponsors to search and view the status of all ERE website submissions.

The SSA Teacher Questionnaire (Form SSA-5665) is available for download as a PDF document from the website.
