- Includes recipients living in a private, state-certified nonmedical facility established and maintained for the purpose of providing personal care and services to aged, infirm, or handicapped persons.
- Level I (for state Code H): Licensed for fewer than six residents.
- Level II (for state Code I): Licensed for six or more residents.
