Information about the Youth Transition Exploration Demonstration (YTED) Survey

What is YTED? You signed up for the Youth Transition Exploration Demonstration (YTED) about a year ago. YTED is a program offered by the Pennsylvania Office of Vocational Rehabilitation (OVR) that helps people get jobs and training for their future.

What is the survey about? The survey asks questions about your education, work, training, health, and services. It lasts about 20 minutes.

How do I take the survey? You can take the survey in two ways:

  • Online: Scan the personal QR code or use the personal link sent to you by mail and email.
  • By phone: Call 1-844-799-0569

What do I get if I take the survey? You’ll receive a $50 Visa gift card by mail after you complete the survey. Completing the survey will also help improve services for other people.

  • You can skip any question you don’t want to answer.

Why is the survey happening?

  • This survey is part of a study that Mathematica, a research company, is doing with OVR and the Social Security Administration (SSA).
