Advance Designation of Representative Payee Notice

This notice serves as an annual reminder to beneficiaries of their advance designation(s) for review and accuracy.

These letters confirm receipt of application for benefits.

Appointment Confirmation Notices

These notices confirm scheduled appointments with local field offices.

Benefit Verification Letter

This letter shows if a person receives benefits or submitted an application.

Campos Settlement Notices

This notice is part of the Campos, et al. v. Kijakazi settlement agreement, SSA agreed to send a one-time notice to individuals with Supplemental Security Income (SSI) overpayments incurred between March 2020 and April 2023 informing them we may waive their overpayments.

Change of Address Notice

This notice tells the beneficiary or representative payee that an address change was reported and advises them to contact us if they didn't change their address.

Cost-of-Living Adjustment Notices

These annual notices tell beneficiaries and recipients whether their benefit payment will increase or remain the same.

Garnishment Notices

These notices tell beneficiaries of any actions taken against benefit payments to pay the beneficiary's obligation for child support, alimony, or court ordered victim restitution. These notices are provided to both the beneficiary and the court or agency ordering the garnishment of benefit payments.

Medicare Part D Closeout Notice for Written or Oral Inquiries (Intent to File)

This notice closes out an expressed intent to apply for Medicare Part D Extra Help Low-Income Subsidy (LIS) when a scheduled appointment is rescheduled or changed. This notice also closes out a written or oral indication of potential entitlement for Medicare Part D Extra Help Low-Income Subsidy (LIS) or an expressed intent to file for LIS when a scheduled appointment to file is rescheduled or changed.

Recovery of Overpayment Collection Notices

These notices are sent to debtors who aren't receiving benefits (non-pay status).

Request for Evidence of Wages Notice

This notice requests proof of earnings to correct earnings record.

Retirement, Disability, and Medicare for Written or Oral Inquiries (T2 and T18 Closeout Notices)

These notices inform applicants of the need to file an application for Social Security benefits within 6 months after the date of the notice.

Social Security Benefit Statement (SSA-1099/1042)

The SSA-1099 and SSA-1042S forms (Social Security Benefit Statements) are used to determine if the beneficiary's benefits are taxable and to determine the amount the beneficiary is required to report to the Internal Revenue Service (IRS).

SSI Closeout Notice for Oral Inquiries and Protective Filings

This notice informs the applicant of an informal decision about eligibility for SSI.

These notices are mailed to SSI beneficiaries of transitional age (14-17), and their representatives, to tell them about services and supports available to help as they prepare to become adults.
