What to do during a Disability review

When we do a Disability review, you must submit a report.

We sometimes check to see if you still have a disability. This process is called a Continuing Disability Review.

When it’s time to review your Disability case, we’ll mail you a letter asking you to fill out a report. We’ll ask questions about your health and daily life.

Upload your report

Sign in to your account, then:

  1. Search for Continuing Disability Review Report (SSA-454-BK).
  2. Download the PDF and fill it out.
  3. Upload and submit the completed report.

Fax or mail us your report

Print out Continuing Disability Review Report (SSA-454-BK) (PDF). Then, fax or mail the completed form to the Social Security office closest to your home.

After you submit your report

We’ll look over your Continuing Disability Review Report to make sure we have all the information we need. Then, we’ll send your case to an examiner at your state’s Disability Determination Services (DDS). 

The examiner will contact you. They might ask you to fill out more forms or schedule a medical exam.

When we finish our review, we’ll mail you a letter with our decision. If we decide you are no longer eligible for Disability benefits, we’ll also tell you how to appeal.
