Submit an Electronic Request for Access to Your Records
These instructions explain how you can submit a written request electronically on our website for access to records related to your claim(s) for benefits from the Social Security Administration.
Once you verify your identity online, you may electronically complete and submit the request for access to your records.
To complete an electronic request for access to your records:
- You must be the record holder. We cannot accept electronic requests from someone completing the form on behalf of the record holder.
- You must be 18 years or older.
- The records you are seeking must be on the list below.
You may submit an electronic request for access to the following record types:
- Social Security benefit amount(s)
- Supplemental Security Income payment amount(s)
- Medicare entitlement
- Medical records from your claims folder(s)
- Benefit or payment application(s)
- Award notice(s)
- Denial notice(s)
- Appeal request(s)
- Other Social Security records from the following systems of records: Claims Folders System (60-0089), Master Beneficiary Record (60-0090), Supplemental Security Income Record and Special Veterans Benefits (60-0103), and Electronic Disability (eDIB) Claim File (60-0320).
